Primary Care Services Coordination and Development Program

The Department of Health and Human Services (HHS) has recently announced it is soliciting applications for its Primary Care Services Coordination and Development program. This initiative aims to facilitate the recruitment of providers while optimizing the delivery of care by creating strategic partnerships with public policy officials in order to further systemize and advance efforts in health care reform. HHS anticipates allocating $11,000,000 amongst 54 beneficiaries, awarded through cooperative agreements with government agencies, public, or nonprofit organizations operating solely within one state. Awards will be up to $444,379 per year forĀ five years. Submissions are due December 31, 2013.
