Community Health Network Receives $1.5 Million HRSA NEPQR Award

Community Health Network has been awarded nearly $1.5 million through a cooperative agreement with the Health Resources Services Administration (HRSA) for its Interprofessional Collaborative Process Improvement Project. The project, funded by HRSA’s Nurse Education, Practice, Quality and Retention (NEPQR) Program, will improve patient outcomes and strengthen the health care workforce by combining team-based care and evidence-based performance improvement strategies. The overall goal is to achieve the triple aim of better care, better health, and lower costs for diverse populations through a nurse-led model of interprofessional collaborative process improvement. CHNw is partnering with the Purdue University Discovery Learning Research Center for formal evaluation. Words for Good provided extensive consultation in addition to writing and submitting the application earlier this year.
