CMS Million Hearts® Opportunity

The Centers for Medicare & Medicaid Services (CMS) is accepting applications for its Millions Hearts®: Cardiovascular Disease Risk Reduction Model (“Million Hearts”), which supports prevention of cardiovascular disease (CVD), improved health outcomes, and health care cost savings through systematic implementation of beneficiary risk calculation and stratification. The model is an opportunity for healthcare professionals to design sustainable models of care that help reduce 10-year atherosclerotic cardiovascular disease (ASCVD) risk to prevent heart attacks and strokes for tens of thousands of eligible Medicare beneficiaries. The Model will incentivize a diverse range of practices, varying in size, patient case mix and demographics, to calculate risk for all eligible Medicare beneficiaries by using the American College of Cardiology/American Heart Association (ACC/AHA) Atherosclerotic Cardiovascular Disease (ASCVD) 10-year pooled cohort risk calculator and to develop risk modification plans based on beneficiary risk profiles.

The model will use a randomized controlled design. Approximately 720 practices will be selected from the pool of eligible applicants. Approximately 360 intervention and 360 control group practices will be selected from the final pool of eligible applicants. Words for Good is experienced with CMS Model applications and has helped clients procure successful State Innovation Model and Medicare Care Choices Model awards.
